Community First Support Services (CFSS)

At AMANI HEALTH CARE SERVICES LLC, we believe people should have meaningful choices about the support they receive and how they live in their homes and communities.
Community First Services and Supports (CFSS) is a Minnesota health care program designed to give eligible individuals greater flexibility, independence, and control over their supports. CFSS replaced the Personal Care Assistance (PCA) program and Consumer Support Grant (CSG), providing new options for directing care and selecting supports.

What Does CFSS Cover?
CFSS supports many of the same essential needs previously covered through PCA, including:
Activities of daily living (ADLs): Eating, bathing, grooming, dressing, transferring, and other everyday activities.
Health-related tasks: Assistance with certain health-related activities as allowed under the program.
Instrumental activities of daily living (IADLs): Shopping, cooking, laundry, and assistance with self-administered medications.
Observation and redirection: Support for individuals who may need assistance with behavioral needs.
The specific services and amount of support available depend on the individual’s eligibility, assessment, and approved service plan.
More Choice and Control
CFSS gives eligible individuals greater flexibility in how they receive and manage their services.
There are two CFSS service delivery models:
Agency Provider Model
Under the agency model, the individual chooses a CFSS provider agency. The agency serves as the employer of the support workers and is responsible for recruiting, hiring, training, supervising, and paying workers.
The individual remains involved in directing their care and has a voice in selecting their support worker.
Budget Model
Under the budget model, the individual is the employer of their support workers and receives an approved budget to help manage their services.
The individual takes greater responsibility for recruiting, hiring, training, and supervising workers and selects a Financial Management Services (FMS) provider to assist with payroll and related employer responsibilities.
Both models are designed to give individuals meaningful control over their care while offering different levels of responsibility.
Who May Be Eligible for CFSS?
CFSS eligibility generally follows the eligibility requirements for PCA. An individual must meet applicable program requirements, including:

Live in the community rather than a facility such as a skilled nursing facility.

Be able to direct their care or have an authorized representative who can direct care.

Be enrolled in an eligible Minnesota health care program, such as Medical Assistance (MA), an MA waiver, the Alternative Care program, or certain MinnesotaCare coverage.

Complete an assessment that determines eligibility for services.

Meet other applicable Minnesota program requirements, including restrictions related to the Family Support Grant.
Eligibility and available services are determined through the applicable Minnesota assessment and authorization process.
CFSS Can Offer Additional Flexibility
Depending on program rules and individual circumstances, CFSS may allow participants to:
Choose a spouse or the parent of a minor as a support worker.
Serve as a support worker for another person who receives CFSS.
Purchase certain goods and services that support independence.
Purchase a Personal Emergency Response System (PERS) for backup support.
Use an available worker training and development budget to support training based on the individual’s needs.
Receive education and assistance from a consultation services provider when developing a service delivery plan.

CFSS is built around person-centered care. In either service delivery model, the person receiving services can participate in directing their care, selecting workers, and developing their service delivery plan.
